Software Quality Assurance Forums: Unable to open Oracle Forms

QuickTest Professional 9.2
QuickTest Professional Oracle® Add-in 8.2.

Instance 1
OA Framework: 11.5.10.6RUP
Oracle Applications Extension: 9.0.3.8.13 - build 1566
Business Components: 9.0.3.9

Instance 2
OA Framework:12.0.4
Oracle OA Extension: 10.1.3- Build 1028
Business Components:10.3.3

Description of issue:

We have 2 instances of the Oracle E-Business Suites. .

I am able to open the forms for first instance (11.5) when it comes to the second instance (12.0.4) i am unable to open the forms. (check the snapshot added in attachment)

The form window is appearing and staying on for ever with out displaying the forms.

Work around:

I have uninstalled “QuickTest Professional Oracle® Add-in 8.2” then I could open the forms (12.0.4).

Have any one faced the similar type of issues.

I have searched the forum but unable to find any post.
