You first need to figure out what exactly you are trying to do. For example:

1. Automate exisiting test assets (manual test cases)
2. Automate testing as part of the new project
3. Add automation to existing product/application (under maintenance)
4. Create Automation Framework
5. Automate a specific type of testing (Smoke, Load, Performence, Regeession, UI etc)
6. Create a Test Plan for an effort
7. Creating a Test Plan for test automation deliverables (testing of automation itself)
etc.

Test Plan is usually written for an effort, not for automation. It may or may not include information on automation among many other things.

Test Automation is an activity, that may or may not be in scope of a particular effort.

You may also need a Test Strategy that will govern your internal approaches to testing, depending on the context.

For test strategy and automation framework look here: http://sqta.wordpress.com/

For test plan, follow the link in the above post.
