Google is a circular "giver". I just used Google advanced search with the following criteria:
with all of the words = templates
with the exact phrase = code reviews

This returned 112,000 hits. Maybe you could go through each of these and we could recommend some when you have determined the below?? Thank you.

It is whatever your organization and/or customer and/or contract require.

Depends on the level of code review. The answer can depend on the type of code review you are talking about (peer review, group review, formal inspection, etc.), but I think the best place to start is by creating a checklist.

Make a list of the types of issues you want to find in code reviews, and how they would be found (if applicable). If, for example, you have coding guidelines, list the guidelines you want the reviewer to check for. You can also list where and how to look for performance or security issues, or whatever.

Morever the Common Template for Code review is :

DATE:
Project Name:
Domain Expert:
Tech Lead / Moderator:
Author:
1 Reviewer:
2 Reviewer:
3 Reviewer:

Code to be reviewed:

Signature..

Hope it'll help you.
