Here is the scenario: I am going to create brand new QA lab for our department. In the nutshell, I know that I will need four to five workstations with Testing tools and applications installed on them. But, how do I document entire process in nice word format? Also, what exactly goes into QA lab? Please provide me solutions if yuo guys have already created on QA lab from scratch.
You need to document the testing Process that you follow your organization in your org specific Process template.
You can include some of the best practices followed for testing in your kind of domain as well as technology collected from the practitiners or from industry.
Generally the process templates cover the below given points:
Process(Activities with responsibility matrix) along with form/template/checklist/guideline/standard hyperlink to be used. This you can have in a tabular format with artifacts and responsibility embedded.

First of a small question: what do you mean by QA lab? From description it seems like this will be automated (or performance) testing lab (which is not the same as QA, but who cares ... [img]/images/graemlins/smile.gif[/img] ). Or maybe there will be some servers (web server, DB, etc) as part of manual test environment?
While you consider your answer here are some generic advices: think if you have to document usage (who and when will have access to each computer, how much disks space, etc.), hardware description (including network), software (dynamic descriptions to be maintained), roles (who in charge, who knows the passwords, etc), ...
