I have adopted the plan suggested in “The Art of Computer Systems Performance Analysis....” by Raj Jain, 1991. Don’t be afraid of the age, the book is outstanding although not so easy to read. The first chapter is exactly about test planning 0 it described in detail the template (although it doesn’t call it this ways – I believe that was not the popular term back then).
I strongly recommend buying a book (this or any other) instead of simply acquiring a template. Templates don’t describe why, only what.
To communicate results start with goals not with the results. The book I mentioned also addresses the result reporting issues. Mostly guided by this book I created also performance test report template specific for my company. However this part required more of my own ideas compare to test plan template.
