Record and run test on any open Windows-based application:
Instructs QuickTest to record and run on any open
Note: Make sure that all the applications on which you
want to record are currently closed. QuickTest can record
on the applications that you open manually only after you
select this option and click OK. Instances of these
applications that are already open when the Record and
Run Settings dialog box opens may be ignored or may not
be recorded correctly.
I have opened windows application before opening qtp itself and it records pretty well.
So it is not necessary to open the application after doing the setting.Have you faced any kind of problems otherwise?

Windows application can be identified using the generic windows API. So whether the application is open before or after doesn't make a difference.
For other application such APIs don't exist and QTP needs to hook its own DLL to communicate with the applications. That is why then need to be launched after QTP is open

Refer HP QuickTest Professional Add-ins Guide -> Using the <Addin Name> Add-in page.
The section Opening Your Application states whether the application can be opened before/after opening QuickTest.
For windows based apps->>>
You can open your standard Windows application before or after opening QuickTest.
Standard Windows testing support is always loaded in QuickTest. It is therefore not an available option in the Add-in Manager.
