Vuser Vs. Database

I have a product filling form. After filling this form and clicking on Add
will update the database immediately. Duplicate products are allowed.
I recorded this actions and i tried running 5 Vusers and the five records
are added to the database.
But when i try with 15 Vusers only 11 or 12 records are getting added.
Sometimes 15 records are added but some are left blank.
I am checking the Oracle database manually. Using LR 6.5
Why is this happening? Am i doing something wrong?
Any help will be appreciated.

Has the "system under test" been functionally tested?
Does it work (write a record to the DB) every time you run your test
manually from a browser and also from the VU generator?
I suspect that maybe you have uncovered a resource limitation in your
"system under test".
Can the developers show you with logs what is happening to each user
request, such that you can help them find out why?
It can be something like there is only ten connections allowed to the
database from the apps server and thus any other concurrent users are
dropped when they should be queued. Just a guess.
