I have set two fields bg_responsible and bg_subject to be required on their screens within defect, but this is still accepting blank and or null values and on the users screens the fields are not shown in red.
Do I need to code in the background to verify these are not null as a user tries to save?

Read the doco that comes with QC, this is pretty well covered in the admin help doco. Both the set up under project entities and workflow can influence the behaviour of fields. If you think that you have it set up correctly under project entities then more than likely your issue in in the workflow.
The workflow script overrides anything you setup in the project entities. So if you marked a field required in project entities, but the workflow script alters that (setfieldapp()) then the workflow script wins.
Also there are some system fields that ignore your setting. For instance you cannot hide some fields or if you make a field required, but hide it... QC is now smart enough to figure that out... that was not always the case
