Are you having some difficulty formatting the data for yourself?
There are only about a half-dozen fields that are required for creating a test case. Those would be the minimum columns you would have to have in your Excel file. Beyond that, what you upload depends on what you need to accomplish. Do you need to upload Step data? Do you have any custom fields that you need to set values for?
What are you looking for in a template? In my opinion, what one would upload is so dependent on how you use QC that a template from another organization is irrelevent.
