It is somewhat odd to hear it stated like this but I would say that strict "middle tier" testing means you are testing the middle aspect of a n-tier architecture. For example, if you have, say, ATG Dynamo between your client and your server, strict "middle tier" testing is just testing the Dynamo aspects. "Three tiers" testing most likely refers to testing all three aspects of the common tier-division. This means testing clients, middleware, and servers. You can also look at it less specifically in terms of hardware and realize that, in general, a three-tier architecture has a presentation tier, a middle tier, and a data tier.
