i want to know what are the different formats (pdf, doc, excel, quality center) the requirments are given to QA team.

Where I've worked, it's always doc
...but it could be any of the above, plus anything else. It doesn't really matter what they are written on.
Same, usually a Word doc but it could be any of those and any others and will depend whatever your firm has decided to use.

I've also worked in shops where they came on serviettes, bar coasters and written on the back of other documents.
As with many things, it depends on the norms of the individual organization.
