Defect Density is the measure of how many defects were found in a specific release of an application.
Defect Rate is a measure of the rate at which defects are found, such as 3 defects per hour.

Defect Density is calculated using the formulae Number of defects/Function Points (or) KLOC(Kilo Lines of Code)
Defect Rate is nothing but the flow of the defects as Rich said.
