I am looking for a good/comprehensice list of Defect Root Causes that I can implement for my testing project.
If you have a good and comprehensive one, please do share?
Wouldn't the root cause of a defect be rather project-and-company specific?
Or are you looking for really generic terms like:
- mistake in code
- mistake in requirements

Agreed with @Joe, it had to be specific to your environment and processes followed.
However have you heard about orthogonal defect classification?
You can check Activity-Trigger mapping in this link of IBM for starting points. Orthogonal Defect Classification
