Wow - no offense, but have you even tried to do a simple search out with all the vendors listed on this site?
There are more an a couple of tools all listed and linked from this site that cover those topics.
Additionally, there is a lot to consider than just the tool to see if it is appropriate for your situation. Your environment, your development tools, your process (or lack of) currently in place, budget, experience level of your group, commitment from Mgmt for training and support, and the list can go on.
Might want to look at the companies listed on this site and delve into the toolsets they offer up and start narrowing down what fits your situation. Then contact each one for demonstrations, and prcing, etc.
